Covid-19 Screening and Safety Measures

You will be asked to complete a COVID-19 symptom screening questionnaire prior to treatment, and to review and sign a waiver.

You will be asked to register at the reception desk upon arrival at the clinic and to notify us when you leave.

You will be required to clean your hands with disinfectant when you enter the clinic.

Wearing a mask will be mandatory within the clinic; we invite you to bring your own mask.

The waiting room will not be open. Please wait for your appointment start in your vehicle in the parking lot. We will inform you as soon as the staff is ready to receive you.

Accompanying persons, such as spouse, companion and/or children, will not be accepted to enter the clinic.
